This topic is called you and your genes, and almost everything difficult about it is hidden in that second word. ⚠️⚠️ A GENETIC RESULT ABOUT YOU IS NEVER ONLY ABOUT YOU. Have a test, and what comes back is partly a statement about the people you got those genes from and the people you might pass them to.YOUR PARENTS. YOUR BROTHERS AND SISTERS. YOUR CHILDREN, INCLUDING ANY YOU HAVE NOT HAD YET. None of them sat down, read a form and agreed to anything. That is not true of most medical tests. A broken bone is yours. A blood pressure reading is yours. ⚠️ A GENETIC RESULT IS THE ONLY ORDINARY TEST WHOSE ANSWER IS PARTLY ABOUT PEOPLE WHO WERE NOT TESTED. ⭐⭐ AND THAT BREAKS THE USUAL RULE FOR DECIDING THESE THINGS, WHICH IS THAT A PERSON DECIDES ABOUT THEIR OWN BODY. HERE, THE INFORMATION IS SHARED, SO THE DECISION IS NOT CLEANLY ANYBODY'S ALONE. Follow that through and the whole of this topic lines up behind one question.TESTING. If a test tells you something, it has told you something about your sister too. Does she have a right to know? Does she have a right NOT to know, if she would rather not? ⚠️ AND THOSE TWO RIGHTS CAN BELONG TO DIFFERENT PEOPLE IN THE SAME FAMILY AT THE SAME TIME.INSURANCE AND EMPLOYMENT. When people object to genetic results being used in those decisions, the objection is sharper than general unfairness. ⚠️ IT IS THAT INFORMATION ABOUT A FAMILY WOULD BE USED TO MAKE A DECISION ABOUT ONE INDIVIDUAL. ⭐⭐ AND CHANGES THAT ARE PASSED ON. Some proposed techniques would change only the person treated. Others would change something inherited, so the change continues into people who do not exist yet. ⚠️ THOSE PEOPLE CANNOT BE ASKED, NOT BECAUSE ANYONE FORGOT, BUT BECAUSE THERE IS NOBODY THERE TO ASK. Many who argue about this treat that as a difference in kind rather than of degree. One question to carry through everything that follows: whose information is this, and who was not asked? ⚠️ AND ONE THING THE MODULE WILL NOT DO. IT WILL NOT TELL YOU WHICH ANSWER IS RIGHT.YOUR COURSE MARKS THE QUALITY OF THE ARGUMENT, NOT THE CONCLUSION, AND PEOPLE WHO HAVE THOUGHT CAREFULLY ABOUT THIS DISAGREE.

A result that arrived for two people

Someone is tested and the result also implies something about a sibling who has said they would rather not know. The tested person wants to tell them. What makes this genuinely difficult?

  • Two real claims are in conflict: one person's wish to act on information that is partly theirs, and another's stated wish not to have their life changed by news they did not seek
  • It is simply a breach of medical confidentiality and so is straightforwardly wrong
  • The result may not be accurate enough to act on
  • It should just be left to the doctor to decide

Two that follow from shared information

Arguing it without taking sides badly

You may already have met the point that evidence can tell you what is possible and cannot tell you what ought to be done. ⚠️ THIS IS WHAT TO DO ONCE YOU HAVE ACCEPTED THAT AND HAVE TO WRITE SOMETHING. ⭐⭐ START BY NAMING WHO THE DECISION IS ACTUALLY ABOUT. In almost every case on this topic the answer is more than one person, and identifying the second and third parties is the move most answers skip. A shape that works on nearly any stem here: SAY WHAT THE TECHNIQUE OR TEST ACTUALLY DOES. One sentence, accurately, without drifting into how genes work. NAME EVERYONE AFFECTED, including people who were not tested and people not yet born where that applies. ⭐ SAY WHAT EACH OF THEM COULD REASONABLY WANT, and give the strongest version of each position rather than the weakest. ⚠️ AN ARGUMENT YOU HAVE MADE EASY TO DEFEAT HAS NOT BEEN ANSWERED. SAY WHICH CLAIMS CONFLICT AND WHY THEY CANNOT ALL BE MET. THEN GIVE A JUDGEMENT, AND SAY WHAT WOULD CHANGE IT.THAT LAST CLAUSE IS THE ONE THAT LIFTS AN ANSWER: a conclusion you can say the conditions for is a reasoned one, and a conclusion nothing could alter is a preference. ⚠️ THREE HABITS THAT COST MARKS ON THIS TOPIC. ⚠️ THE FIRST IS THE LIST OF ADVANTAGES AND DISADVANTAGES. Two columns, balanced, ending with the observation that there are arguments on both sides. It is accurate and it is not an argument. ⚠️ THE SECOND IS TREATING ONE SIDE AS OBVIOUSLY UNREASONABLE. People who have thought hard about this disagree with each other, and an answer that cannot state the other view fairly will not be trusted with a judgement. ⚠️ AND THE THIRD IS REACHING FOR A FACT YOU DO NOT HAVE.A PROBABILITY, A TEST ACCURACY, A COST, OR A CLAIM ABOUT WHAT THE LAW CURRENTLY ALLOWS: IF YOU ARE NOT CERTAIN, ARGUE FROM THE STRUCTURE INSTEAD. ⚠️ THE LAW IN PARTICULAR VARIES BY COUNTRY AND CHANGES, SO A CONFIDENT CLAIM ABOUT WHAT IS PERMITTED IS EASY TO GET WRONG. And write about people as people. The subject of this topic is not a technique; it is somebody's family.

One result, four people

Here is a situation of a kind that genetic counsellors meet often. No condition is named, because the shape of the problem is the same whichever one it is. An adult is tested for an inherited variant, having thought about it and agreed to it. The result comes back and it is positive.SO FAR THIS IS AN ORDINARY MEDICAL SITUATION: ONE PERSON, ONE DECISION, ONE RESULT. ⚠️ IT STOPS BEING ORDINARY IMMEDIATELY. BECAUSE THAT RESULT SAYS SOMETHING ABOUT AT LEAST ONE PARENT, who may be alive and may never have been tested, and who may now be able to work out something about themselves from a conversation at a family gathering. IT SAYS SOMETHING ABOUT A SISTER, who has a real chance of carrying the same variant and who said some years ago that she would rather not know. AND IT SAYS SOMETHING ABOUT A CHILD, who is nine, cannot consent to anything of this kind, and whose situation will be a question for somebody in about a decade. ⚠️⚠️ FOUR PEOPLE ARE NOW INVOLVED AND EXACTLY ONE OF THEM AGREED TO ANYTHING. Now notice how each conflict is a version of the same thing.THE TESTED ADULT WANTS TO TELL THE SISTER, AND HAS A GOOD REASON: she might want to make decisions that the information would change. She has an equally good reason for her earlier position: being told would alter how she lives for years, and she chose not to have that. ⚠️ BOTH WANT SOMETHING REASONABLE AND THEY CANNOT BOTH HAVE IT.THE PARENT IS NOT BEING TOLD ANYTHING DIRECTLY AND MAY WORK IT OUT ANYWAY. There is no way to have the conversation with the sister that does not make it available to the parent as well. ⭐⭐ AND THE CHILD IS THE CLEAREST CASE OF ALL. Nobody can ask a nine-year-old for agreement of this kind, so somebody will decide on their behalf, and whatever is decided is a decision about a person who had no part in it. ⚠️ NOW CHANGE ONE THING AND WATCH THE PROBLEM DEEPEN. SUPPOSE A TECHNIQUE EXISTED THAT COULD ALTER WHAT IS PASSED ON, SO THAT DESCENDANTS WOULD NOT INHERIT IT.THE PEOPLE AFFECTED BY THAT DECISION INCLUDE SOME WHO DO NOT EXIST. Some argue this is a reason for great caution, because consent is not merely absent but impossible. Others argue that parents already make irreversible decisions for children who cannot consent, and that refusing to act is also a decision made on their behalf. ⚠️ THIS MODULE DOES NOT SETTLE THAT, AND NEITHER WILL YOUR EXAM ANSWER BE EXPECTED TO. WHAT IT WILL BE MARKED ON IS WHETHER YOU NAMED EVERYONE INVOLVED AND GAVE EACH OF THEM THEIR BEST ARGUMENT.

  • An answer gives four advantages and four disadvantages of a genetic technique, all accurate, and concludes that there are strong arguments on both sides. Why does it not reach the higher bands?
  • A student writes that anyone who objects to a particular technique simply does not understand the science. How would you respond?
  • An answer supports its conclusion by stating confidently what the law currently permits in this area. What is the risk?
