Two lists that are really one

Open the specification at this section and you meet two long lists back to back. A list of NINE groups of people: babies, toddlers, pre-school children, school-aged children, adolescents, adults, older people, and women who are pregnant or breastfeeding. And a list of NINE diet-related health conditions: obesity, cardiovascular disease, coronary heart disease, diabetes, diverticulitis, poor bone health, poor dental health, anaemia and high blood pressure. Eighteen things to learn, apparently, with nothing connecting them. ⚠️ THEY ARE NOT TWO LISTS. THEY ARE ONE IDEA SEEN FROM TWO ENDS. A nutritional need is what a body requires at a particular point in life. A diet-related condition is, very often, what happens when that need goes unmet AT THE STAGE WHERE IT MATTERED MOST. Take one example and the pattern is obvious. Bone is built during childhood and the teenage years, which is why calcium and vitamin D matter so much then. Poor bone health in later life is what a shortfall during those years can contribute to. Same fact, two ends. One appears on the needs list, one on the conditions list, and they are the same story. ⚠️ WHICH MEANS TIMING MATTERS, NOT JUST QUANTITY. A need met at the wrong stage is not the same as a need met. One thing to be clear about before going further. These are conditions with many causes, of which diet is one. Genetics, medication, illness, income and circumstances all play a part. ⚠️ THIS TOPIC IS ABOUT HOW NUTRIENTS WORK IN BODIES, NOT ABOUT JUDGING ANYONE'S, and the exam asks you about the science, not about individual people.

Why does the timing matter

Someone in their sixties is told that their calcium intake as a teenager still matters for their bone health now. Why is that, according to how bone works?

  • Because bone is largely built during childhood and adolescence, so the amount established then affects how much remains decades later
  • Because calcium eaten as a teenager is still sitting in the stomach
  • It does not matter, because only current diet affects current health
  • Because bone health is entirely genetic and diet plays no part

Two things about YOUR specification that revision guides get wrong

Most nutrition revision material online is written for a different exam board, and on two points that will cost you. ⚠️ FIRST, AND THIS IS THE BIG ONE: YOUR SPECIFICATION GIVES NO PERCENTAGES FOR HOW DAILY ENERGY SHOULD BE SPLIT BETWEEN NUTRIENTS. The heading appears; no figures follow it anywhere in the document. Another major board DOES publish specific percentages, and a great deal of revision material repeats them as though they were universal. ⚠️ THEY ARE NOT PART OF WHAT YOUR BOARD ASKS YOU FOR. If a resource has you memorising a set of percentages, check whether it was written for your specification before you spend time on it. ⚠️ SECOND: YOUR SPECIFICATION NAMES NO PARTICULAR FOODS AS ALLERGENS. It refers to foods that may cause an allergic reaction, without listing them. It does name two intolerances specifically, and those two are worth knowing precisely. Learn what an allergy is and how it differs from an intolerance, rather than memorising a list your board never gave you. The same caution applies to the healthy-eating tips your specification mentions. ⚠️ IT NAMES THE FRAMEWORK WITHOUT SETTING OUT THE TIPS THEMSELVES, so know what it is and where it comes from rather than reciting a numbered list from somewhere else. Now the habit that earns marks here. ⚠️ NEVER STATE A NEED WITHOUT THE REASON, AND NEVER STATE A REASON WITHOUT THE GROUP. "Teenagers need calcium" is a fact with nothing behind it. "Teenagers need calcium because bone is still being laid down, and the amount built now affects bone health decades later" is the same fact doing three times the work. The link is what is being marked.

Four needs, four conditions, one pattern

Here is the claim doing actual work. Each of these appears on your specification's needs list at one end and its conditions list at the other. CALCIUM AND BONE HEALTH. Bone is built mainly through childhood and the teenage years, and calcium, with vitamin D to absorb it, is what it is built from. A shortfall during those years can contribute to weaker bones later, which is why poor bone health appears as a diet-related condition even though the diet that mattered most was decades earlier. IRON AND ANAEMIA. Iron is needed to carry oxygen in the blood. Requirements are highest where growth is fast or where losses are greater, which is why adolescent girls and women who are pregnant are the groups usually highlighted. Too little over time can lead to anaemia, with tiredness and breathlessness as the effects most often described. SUGAR AND DENTAL HEALTH. ⚠️ IT IS NOT ONLY HOW MUCH, BUT HOW OFTEN. Each time sugar is eaten, acid is produced in the mouth for a period afterwards. Frequent small sugary snacks therefore do more damage than the same sugar eaten at once, which is why advice to young children is usually about frequency rather than quantity alone. FIBRE AND DIVERTICULAR DISEASE. Fibre gives the digestive system bulk to work against. A diet persistently low in it is associated with problems in the gut wall, which is a condition more common in later life, and it is one reason fibre is emphasised for older people. ⚠️ NOTICE THE SHAPE ALL FOUR SHARE. A nutrient, a group for whom it matters most, a reason it matters THEN, and a condition that can follow if it is missing. Learn the four-part shape and the two lists collapse into one. Obesity, cardiovascular disease and diabetes also appear on your conditions list, and they connect to energy and to the macronutrients. ⚠️ THOSE BELONG TO THE ENERGY BALANCE TOPIC RATHER THAN THIS ONE, so you will meet them properly there. All of these conditions have several causes, of which diet is one.

A nursery serves a baked pasta dish to pre-school children and wants it to better suit them. Assemble a modification that names the change, the route it uses, and the reason.

Three decisions to make

  • A classmate has memorised a set of percentages for splitting daily energy between nutrients, from a revision site. They cannot find them in the specification. What should you tell them?

Explain why one diet does not fit everyone
